NETWORK RICAPPS

Research Network on Chronicity, Primary Care and Prevention and Health Promotion (RICAPPS).

The overall objective of RICAPPS is to promote health outcomes-oriented research in primary care, chronicity and health promotion, in order to transfer the results to clinical practice and health service organisations, and thus ensure the improvement of health care for the population.

RICAPPS is structured into five objectives comprising fourteen work packages (WPs). WP8 is coordinated by the Data Science for Health Services and Policy Research group of the Aragon Institute of Health Sciences (Instituto Aragonés de Ciencias de la Salud).

The RICAPPS network of research in chronicity, primary care and health promotion is made up of 26 research groups, located in 12 autonomous communities and 9 clinical care groups from 9 Spanish autonomous communities.

RESEARCH AREAS

WP 8: Evaluating primary care services and continuity of care in response to chronic diseases

Characterising services, interventions and policies

Characterise services, interventions and policies designed, developed or implemented to address the needs of chronic and frail patients and improve their health outcomes. Within this line of research, the key themes will be the digital transformation of primary care and the adaptive staffing of primary care.

  • Task 1. Establish a digital roadmap to enhance integrated care.
  • Task 2. To assess the unequal distribution of primary care resources in the Spanish Health System.
Identifying, analysing and monitoring unwarranted variations

Identifying, analysing and monitoring unwarranted variations in the care of chronically ill and frail patients. Within this line of research WP8 will study equity in access to effective care; appropriate use of clinical interventions, adherence to treatments, in particular those of multi-morbid, multi-treated patients; quality and safety of care delivery and efficiency of care.

  • Task 1. Establish a digital roadmap to enhance integrated care.
  • Task 2. To assess the unequal distribution of primary care resources in the Spanish Health System.
Analysing care pathways within and between levels of care

Analysing care pathways within and between levels of care. Within this line of research, particular emphasis will be placed on multi-condition, multi-treatment patients and their exposure to primary care, inpatient and outpatient hospital care, emergency care and long-term care.

  • Task 1. Establish a digital roadmap to enhance integrated care.
  • Task 2. To assess the unequal distribution of primary care resources in the Spanish Health System.
Assessing the effectiveness and efficiency of continuity of care

Assessing the effectiveness and efficiency of continuity of care. Within this line of research, the most effective and efficient care pathways will be obtained, those that would optimise continuity of care. Special attention will be paid to patients with multiple conditions and multiple treatments.

  • Task 1. Establish a digital roadmap to enhance integrated care.
  • Task 2. To assess the unequal distribution of primary care resources in the Spanish Health System.
