New Atlas of Potentially Avoidable Hospitalisations by basic health area 2018-2020

This Atlas of Potentially Avoidable Hospitalisations shows indicators for 6 chronic conditions between 2018-2020 by basic health area: Acute Complications of Diabetes (ACD), Chronic Obstructive Pulmonary Disease (COPD), Congestive Heart Failure (CHF), Dehydration (DH), Non-Primary Angina without associated procedure (Angina) and Adult Asthma (Asthma).

This atlas shows the HPE rates for each of the chronic conditions included in the study, thus showing the variation by basic health zone and health area, and the differential management of these types of chronic conditions. It is also complemented with the probability of having more HPE than expected for each of the basic health zones and health areas.
